Transforming Care into Heart Failure: Significant Reductions in the Duration of Admission into Various ICC Gravity Levels and Costs by up to 50% with the Integration of POCUS

Context

This report presents the results of an innovative Quality Improvement Initiative (IQ) carried out by Butterfly Network, in collaboration with Rutgers Robert Wood Johnson Medical School and Robert Wood Johnson University Hospital. The findings were initially presented as part of a last-minute clinical trial in EuroEcho Conference December 2024.

The study focused on the integration of bedside ultrasound (POCUS) in the routine care of inpatients with congestive heart failure (CHF), aiming to evaluate both clinical and economic outcomes.

More than a thousand studies have highlighted the clinical value of POCUS in the management of ICC, but few have been designed specifically to assess its economic impact within an effort to improve quality with the purpose of establishing POCUS as a new standard of care. This initiative sought to fill this gap using a study model. randomized cluster with progressive scaling, reflecting real conditions of care. The study included 216 patients, with hospitalists trained through the Butterfly Academy And with the support of sonographers.

Impact

The study demonstrated the significant impact of daily use of POCUS on patient outcomes and health costs. The main results included:

  • Overall reduction in length of hospital stay (LOS): More than 4 days on average between all patient groups.

  • Patients with lower severity (low acuity):

    • Reduction of median hospital admission 6,7 to 5,6 days (↓ 1.1 day).

    • Cost reduction by approximately 50%.

  • Patients with greater severity (high acuity):

    • Reduction of median hospital admission 39 to 16.7 days (↓ 22.3 days).

  • They were performed Cardiac POCUS and pulmonary ultrasonography (LUS)both contributing to faster hospital discharges — especially among patients of lower severity.

Completion

This case study demonstrates that the integration of bedside ultrasound technology (POCUS) to standard hospital workflows in the management of heart failure can generate significant clinical and economic benefits.

With proven reductions in length of stay and costs, POCUS hardware and software provide a scalable and efficient solution to improve hospital care and patient outcomes — while delivering a clear return on investment to health systems.
